Each case is reviewed by the appropriate government agency individually, and there is no precedent set by previous decisions to reimburse or deny. Currently Medicare does not automatically reimburse users for assistive technology computer input devices. Because there is not currently a national coverage decision by the Health Care Financing Administration (HCFA) classifying assistive technology devices as Medicare reimbursable Durable Medical Equipment, purchasers of the Tracer will need to apply for reimbursement to their individual Durable Medical Equipment Regional Carriers (DMERC) (see below for a complete list).
To apply, an applicant will need to send in ALL of the following:
- A 1490S claim form (available at www.hcfa.gov/medicare/edi/edi5.htm)
- A letter stating the medical need for the item
- A receipt showing proof of purchase
- A doctor's prescription for the Tracer
- A certificate of necessity, which needs to be signed by a doctor and the person applying for the Medicare reimbursement
The certificate of necessity must be requested by the individual seeking Medicare reimbursement. It can be obtained by calling the DMERC that provides coverage for your state (see below for a complete list). Once the application is completed, the individual DMERC will review the documents to determine if reimbursement will be granted.
